Catering industry and group meal delivery unit Health Standards Ministry of Health.20053 requirements3.1 The implementation authority for the health permit of the food production and operation unit at the entry and exit port is the port inspection and quarantine institution authorized by the directly affiliated inspection and quarantine bureau.
3.2 The port inspection and quarantine institution shall publicize the procedures and contents of the health administrative licensing.4 procedures4.1 Application and acceptance
4.1.1 Application
4.1.1.1 The port food production and operation unit shall submit a written report to the port inspection and quarantine institution 30 days before the commencement of business or before the expiration of the health permit.
Application.
4.1.1.2 When applying for the sanitation permit of the port food production and operation unit, the applicant shall submit the following application materials to the port inspection and quarantine institution.
--- "Application for Health Permit for Food Production and Operation Units at Entry and Exit Ports" (see Appendix A);
--- "Notice of Enterprise Name Approval";
--- A copy of the original of the "Business License" (submitted after the acquisition);
---Health management system (including. health cleaning system, health check and health knowledge training system for practitioners, disinfection system, procurement
Institutions, vector biological control systems, etc.);
--- Enterprises that have established management systems submit relevant management system documents;
--- Floor plan of production and operation unit, production process flow chart, flow of people, logistics map, network diagram of upper and lower water pipes, layout of anti-fly and rat-proof facilities;
---Production of raw material formula, production equipment information and product packaging material description;
---Product hygiene standards, product identification, sanitary inspection results of production products and safety and health control measures;
---Production water sanitation inspection report;
---Practitioner health certificate and health knowledge training certificate, returned after review;
--- A copy of the valid ID card of the corporate legal person.
