This standard is in accordance with GB/T Draft the rules given in 1-2009.
This standard was proposed and managed by the National Port Standardization Technical Committee (SAC/TC 530).

Pilot Visa Form
1 Scope
This standard specifies the basic requirements, content and style of pilot visa forms.
This standard applies to pilotage agencies approved by the competent transportation department for use in ship navigation activities along China's coasts, rivers and ports.
Design, printing and use of pilot visa forms.
2 Basic requirements
2. 1 The pilot visa form (hereinafter referred to as the “visa form”) shall be printed in a horizontal version of 191mm × 145mm (32 open), and shall be in both Chinese and English.
2. 2 The header of the visa document shall contain information such as the name of the institution, the name of the document, and the date of the document.
2. 3 The tail of the visa slip shall be valid only after it has been signed by the pilot and the master.
2. The names of the 4 columns shall be marked in Chinese and English.
2. 5 The visa slip should be no less than two copies.

3. 1 Ship name (Ship's Name)
The registered name of the cited ship.
3. 2 Port of registry (Port of Registry)
The port city where the owner of the cited ship has registered the ownership of the ship.
3. 3 IMO Number (IMO Number)
The serial number issued by the Lloyd's Register of Shipping, commissioned by the International Maritime Organization (IMO), consists of 7 Arabic numerals.
3. 4 gross tonnage (Gross Regulatory Tonage)
The gross tonnage indicated on the ship's tonnage certificate is abbreviated as GRT.
3. 5 net ton (Net Registred Tonage)
The net tonnage indicated on the ship's tonnage certificate is abbreviated as NRT.
3. 6 Ship's total length/width (Length Overr All/Beam)
The maximum length of the ship and the maximum width of the ship in the ship registration certificate are abbreviated as LOA/Beam, and the unit is meter (m).
3. 7 Draught
The maximum vertical distance from the front and rear bottom of a ship to the point where the hull is connected to the water surface

3. 8 Agent Company/Shipping Company
The agency or shipping company that filed the application for pilotage.
3. 9 Import (IN)
The act of a ship entering a port.

3. 10 Relocation (SHIFT)
The movement of a ship from anchorage A or berth to anchorage B or berth in the port.
3. 11 Exit (OUT)
The act of a ship leaving a port.
3. 12 International Trade (Internatioal Trade)
The cited ship sailed on international routes.
3. 13 Domestic Trade (Domestic Trade)
The cited ship sailed on domestic routes.
3. 14 Pilotage Location (Area Pillowing)
The place where the pilot starts and the place where the pilot ends are collectively referred to as the pilot location and are related to the collection of pilot fees.
3. 15 Start time of pilot work (Tim Pillotage Commenced)
The time when the pilot starts the pilot work after boarding the piloted vessel is the start time of the current pilot charge.
3. 16 Pilot Work Completion Time (Tim Piolet Complaint)
The time at which the pilot ends the pilot's pilot mission.
3. 17 Remarks (Remarks)
Add necessary notes and explanations to the current visa slip.
3. 18 Pilot's Signature (Pilot's Signature)
Signed as the pilot of the pilot.
3. 19 Signature of the Captain (Master's Signature)
Signature of the captain of the cited ship.
4 style
The visa form is a proof of pilot service provided by the pilot. See Appendix A for specific forms.
